The Radio Active Zombie was created the day his live body was disposed of in a drum of radioactive waste and throw into a deep-water river.  Why would anyone put a man through this fate you ask, well lets just say that our poor victim was planning to tell the authorities about the local power plants disposal practices. 

Unfortunately, the perpetrators didn’t consider that the radioactive gunk would reanimate our Good Samaritan, something they realized when the Radio Active Zombie got its revenge.
